The Portugal Passenger Locator Form is an online health declaration introduced in Portugal under the Safe and Clean Program to help the Portuguese health authorities keep the Covid-19 situation within the country under control.
Every incoming visitor to Portugal must fill out the Portuguese Passenger Locator Form within 4 days of the planned arrival in the country.
The PLF is available for travellers online and can be easily completed using a phone, laptop, or any other device connected to the internet.
Completing the Portugal Passenger Locator Form results in a PDF confirmation issued and delivered to travellers via email along with a unique QR code for each visitor.
Requirements to complete the Portugal PLF
Completing the Portugal Passenger Locator Form takes place online making the requirements to meet minimal.
Travellers applying for the Portugal PLF must have:
- An active email addresst
- Internet connection
- A valid passport
- Address for the stay in Portugal
- Flight details
- Other essential information about their trip to Portugal
The PLF for Portugal can be completed online using any preferred working device, including a phone, laptop, PC, and tablet.
How to apply for the Portugal PLF online
To apply for the Portugal PLF online, first make sure that you have all of the required details to complete the form with, including the address for the stay in Portugal and all essential flight-related details.
Next, within 96 hours (4 days) start completing the Portugal PLF application form with the following data:
- Your personal details (name, date of birth, gender)
- Passport number
- Contact details (email address, phone number)
- Flight information (airline, flight and seat number, date and time of arrival, country of departure, specific destination within Portugal)
- Stay in Portugal details (address for the stay – city, postal code, street name, etc.)
- Address in your home country
- Emergency contact information (name and phone number)

What happens after applying for the Portugal PLF

The PDF confirmation of completing the Portugal PLF will contain a unique QR code for each traveller who has submitted their Passenger Locator Form.
Once you receive the PDF version of your Portugal Passenger Locator Card, download the document and either print it out or keep it in an electronic form to then present it to the customs officials upon arrival in Portugal.
Do children need to complete the Portugal PLF
Children and all underage travellers must also have a Passenger Locator Card for visiting Portugal.
