What is the Passenger Locator Form for Netherlands / Public Health Travel Declaration?

The Passenger Locator Form (PLF) for the Netherlands is a travel document that should be filled by anyone who is travelling to the Netherlands, and it should be done at least a day before entering the country.
The purpose of the Passenger Locator Form is to contact you if a person who has travelled with you develop COVID-19 symptoms and to collect details on your health conditions such as whether you had COVID-19 like symptoms before the departure.

Note: You should remember that providing false information in this document is a criminal offence. Therefore, you should be honest in providing information for this form and you shouldn’t falsify your COVID-19 test results.

How should you apply for the Passenger Locator Form for the Netherlands?

This Health Declaration Form can be completed entirely online and it will only take a few minutes of your time.
The following requirements should be provided before you are submitting the Passenger Locator Form for the Netherlands;

1. Health information about yourself and your travel companions on whether they are experiencing any of the symptoms related to COVID-19.
2. Travel details for the last 10 days before travelling to the Netherlands, on whether you were in contact with someone who was infected with COVID-19.
3. Other information such as the flight number, seat number, the date of birth and the surname of the passengers.
4. A vaccination/recovery certificate for those who are travelling from within the EU to the Netherlands. The accepted vaccines are Pfizer BioNTech, Moderna, AstraZeneca, Johnson & Johnson, Sinovac and Sinopharm.
5. A negative PCR test result for those who are travelling from a high-risk area for COVID-19 or outside the EU, and it should be taken no more than 48 hours before arriving in the Netherlands (This is only applicable for travellers who are 12 years old and above).

It is better to keep a printed copy of the passenger locator form so that you can present it at the airport upon arrival in the Netherlands.

Medical Requirements To Enter Netherlands

• Every traveller who is 13 years old and above should complete a Passenger Locator Form and Health Declaration Form before arriving in the Netherlands.
• Those who are travelling from high-risk COVID-19 areas should be self-quarantined for 10 days in the Netherlands, and this should be done even if you have a vaccination certificate indicating that you have taken both doses 2 weeks before travelling to the country.
• However, you can get out of being quarantined if the PCR test result is negative on the 5th day of arriving in the Netherlands.
